What to expect in the first week after a knee injection
Expect mild aching, warmth or a feeling of fullness in the knee for one to two days after an intra-articular injection, settling with rest and ice. Walk normally but avoid running, deep squats, stair training and heavy lifting for 48 hours. A corticosteroid usually starts working within one to three days; hyaluronic acid builds over two to six weeks. Call the clinic urgently if the knee becomes hot, markedly swollen and increasingly painful over 24 to 48 hours, particularly with fever.
The injection itself takes under a minute. The week afterwards is where most of the questions are, and most of them have the same answer: this is normal, give it time.
The first few hours
The knee often feels full or tight. That is the volume of fluid you have just added to a closed space, and it settles as the material distributes. If local anaesthetic was used, the joint may feel unusually good for an hour or two and then return to baseline. That is the anaesthetic wearing off, not the treatment failing.
You can walk out and go about the day. Arrange a lift only if both knees were injected or the leg feels unsteady.
The first 48 hours
- Walk normally. Movement helps the material distribute through the joint.
- Avoid running, jumping, deep squats, stair training and heavy lifting.
- Ice for fifteen minutes at a time if the knee aches, with a cloth between the ice and the skin.
- Ordinary painkillers are fine unless your doctor said otherwise.
- Avoid swimming pools, hot tubs and long baths while the puncture site closes.
A flare is not a failure
A minority of people get a day or two of increased pain before the improvement arrives, most often after a corticosteroid. It settles on its own. The distinction that matters is direction: discomfort that is improving by day two is expected, discomfort that is worsening by day two is not.
When you should feel a difference

The slow ones are slow because they are not painkillers. Hyaluronic acid restores the property of the joint fluid rather than blocking a pain signal, and that takes time to translate into how the knee feels. People who expect an overnight change often conclude at day five that it has not worked, when it has simply not started.
Use the window
This is the part that separates a good result from a short one. The comfortable months after an injection are the best opportunity you will get to build the thigh muscle that protects the joint. Start the strengthening work once the initial soreness settles, usually at around a week.
When to call the clinic
- The knee becomes hot, markedly swollen and increasingly painful over 24 to 48 hours.
- You develop fever, chills or feel generally unwell.
- There is spreading redness around the injection site.
- You cannot bear weight at all.
- Six weeks have passed and you have felt no change whatsoever.
Septic arthritis
Infection inside a joint after an injection is rare, and it is an emergency when it happens. The pattern is unmistakable: pain that gets worse rather than better after the second day, with heat, swelling and often fever. Do not wait for a scheduled appointment. Go the same day.
